We understand that arriving at a doctors office for the first time can be nerve racking. Filling out paperwork, learning  new faces and names, and getting to know your way around the office can be an overwhelming experience.

That's why at Family Tree Chiropractic our aim is to  remove any  stressor causing unnessesary tension and to make you feel comfortable. In this area of the website you will be able to take care of items necessary for your visit prior to arriving, in the comfort of your own home.

Insurance verification:

We have prepared this area to help you better understand the complexities of chiropractic insurance.  We realize how confusing it can be.  To begin, we would like to highlight a misconception--chiropractic insurance was not designed to pay for all chiropractic care.  Most contracts have limits and/or various degrees of co-payment.  

The treatment recommended by our office is never based on what your insurance company will pay. Our recommendations are based on your examination and x-ray findings and on you achieving the maximum spinal correction and thus greater health. Your treatment should not be governed by your insurance contract, and we will not allow the insurance companies to dictate the quality of care we provide to our patients. Your spinal health and thus overall health is our primary concern.

If you do have health insurance which may cover a portion of your chiropractic care, we strongly encourage you to personally contact your carrier. We will do the same, but only as a courtesy to you. Please realize that you are ultimately responsible for all outstanding charges.

To assist you in determining your chiropractic benefits, we suggest that you ask the following questions.

What is the name of the insurance company employee that you are speaking with? Date and Time?

Is my plan a PPO or HMO? Is Family Tree Chiropractic/Dr. David Parker part of my insurance network?

Do I need a referral for chiropractic care? If you do, you must first see your primary care physician for a referral before any charges can be submitted to your insurance company.

How much is my co-pay? Is that per visit or per service?

Do I have a deductible? Amount? Separate chiropractic deductible? Amount? Does my family have a deductible? Amount?

How much of my individual/ chiropractic/ family deductible has been met?

Are adjustments covered? You may ask, “Do you cover a 98941 treatment code?” How many adjustments are covered and at how much?

Are extremities covered? You may ask, “Do you cover a 98943 treatment code?”

Are re-exams covered? You may ask, “Do you cover a 99213 treatment code?”

Are therapeutic exercises covered? You may ask, “Do you cover a 97110 treatment code?”

Are x-rays covered? You may ask, “Do you cover a 72010 treatment code?” If so, how frequent and how much?

Are orthotics covered? You may ask, “Do you cover a L3020-RT and L3020-LT treatment code?”

Are Cervical Pillows covered? You may ask, “Do you cover a LO943 treatment code?”

Do I have chiropractic coverage for my children? List any and all limitations.

Does the effective date of my insurance plan run with the calendar year?
